Job Description & Requirements Specialty: Case Management Discipline: RN Duration: 12 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract SKILLS: 2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment - required clinician must have current work history within 2 years. Computerized documentation, required. Ability to organize and participate in family meetings Participate in the development of the treatment plan. Experience with HMOs, IPAs, etc. Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently.
