Structural Heart Program Coordinator RN opportunity is full time at
40 hours per week, offers full benefits and a convenient day shift
schedule. We are looking for a dynamic individual who coordinates
and oversees the structural heart program throughout the care
continuum from referral through follow-up post procedures. Assess
and evaluate the overall clinical picture and needs of the
Structural Heart Program patient population. Functions in a
multidisciplinary team to facilitate an efficient workflow and
coordinate support of the Structural Heart Program to provide high
quality patient care for inpatients and outpatients. Provide
professional nursing support to the Cardiac Surgeons,
Interventional Cardiologist, and the Structural Heart Team. Job
Duties/Responsibilities: - Identifies and coordinates for patients
with heart valve disorders. Identifies individuals and recruits
appropriate patients into program. Provides symptom assessment and
nursing management, education, care coordination, and resource
allocation as identified by continuous needs assessments. Receives
and reviews referrals for patient appropriateness. - Coordinates
diagnostic workup by determining which tests need to be completed
and assisting patients with navigating the organizations and
departments. - Assists in providing volume and quality reports,
tracking trends and identifying quality improvement initiatives.
Assists in managing the valve data collection/registry. - Conducts
in-services and educational activities with staff, as needed. Keeps
abreast of latest technologies in heart valve repair and integrates
best practices. Acts a key contact with patients by coordinating
care, providing patient education, and serving as the connection

Qualifications - RN license in State of Nevada - ACLS and BLS
required - Three (3) years of clinical nursing experience in
