Job Description:

qualified Physician - OB/GYN. City: Grass Valley State: CA Start

RATES:Malpractice Must be included in rates. Orientation 170 - half
the hourly/callback rate kicks in after 8 hours. Per hour
oforientation or training beyond 8 hours On Call Daily under 3000 -
Per 24 hours to include 4 hours gratis24 Hour Holiday Call Rate -
under 4500 - Per 24 hours to include 4 hours gratisCall Back under
340 Per hour ofpatient contact hours after gratis hours are
completed gratis prorate to MSAterms. Holiday Call Back under 510
Per holiday hour of patient contacthours after gratis hours are
completed gratis prorate to MSA terms. Average worked Callback
Hours: 4-6h Schedule/Availability Requirements:OCT 15-17Facility
Location: Grass ValleySetting: Hospital setting with a dedicated
Family Birthing Center LampD floor which has its own OR for
C-sections. Duties: Deliveries average 23-30 deliveries per month
C-sections ED call coverage for OB/GYN related issues. Required
Procedures: All OBGYN. Specific procedures TBDEMR: CernerSupport
Staff: Anesthesiologists 1 on-call after hours and on weekends for
OR s and FBC General Surgeons 3 local OB/GYN s 1 Family Medicine OB
Pediatrician on-call experienced and skilled Respiratory
Therapists. Reason for Coverage: Cancellation and maternity leave
Travel Preferences:Preferred Lodging: GoldMiner's Inn Credentialing
Information:Timeframe: Min 10 days from completed appRequirements:
Current certification or active participation in the examination
process leading to certification in obstetrics and gynecology by
the American Board of Obstetrics and Gynecology or the American
Osteopathic Board of Obstetrics and Gynecology orSuccessful
completion of an ACGME- or AOA-accredited residency in obstetrics
and gynecologyANDDemonstration of performance of at least 100
gynecological surgical procedures in the past two years or
successful completion of a hospital-affiliated formalized residency
or clinical fellowship.Credentialing with other facilities? No
Additional Details / Questions Answered by Facility:Our current
volume ranges from 25-40 deliveries a month. Special Procedures:
Successful completion of an approved recognized course when such
exists or acceptable supervised training in residency fellowship or
other acceptable program and demonstration of indications for the
procedure/test/therapy and documentation of competence to obtain
and retain clinical privileges as set forth in Departmental
policies governing the exercise of specific privileges. Proctoring
Requirements: Proctoring is required for all appointees having
interval or new privileges in accordance with the specific
proctoring requirements of the relevant departments Refer to the
Proctor Program Guidelines. Additional proctoring may be required
for specific privileges.1OB Proctoring shall include concurrent
observation of: a the first two 2 non-emergency deliveries by
different observers if possible b if Provisional member is granted
privileges for C-Section one of the two 2 deliveries observed must
be a C-Section.2 GYN Proctoring shall include concurrent
observation of first three 3 cases one 1 hysteroscopy one 1
operative laparoscopy and one 1 open laparotomy3 Retrospective
review of three 3 months is required as a minimum. Initial

fields performing operative and other procedures please attach a
listing and summary of your operative and invasive procedures for
the past 24 months. Please include on the listing the name and date
of the procedure admission and discharge date and where performed.
Applicants recently completing a residency or fellowship should
attach a copy of the residency/fellowship logs of procedures
performed and provide a letter from the program director attesting
to current demonstrated competency for the privileges requested.
