LanceSoft is seeking a travel nurse RN Home Health for a travel nursing job in Martinez, California. Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 04/06/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Req: KAISJP00249063 Shift: 08:00 AM to 05:00 PM Shift Notes: Mon- Fri 08:00 AM to 05:00 PM Description: Setting: Home Health Palliative Care Shift Notes: Mon-Fri, 8 hour days 3 days visiting patients (5 visits a day) and 2 days of phone triage. Basic assessment and reporting an SBAR to the practitioner or MD with results of visits. Coverage Area: Will work onsite in Martinez and visit patients in their homes, SNFs or Assisted Living. Experience: Minimum 1 year HH experience in the last 3 years Job License/Certification requirements: CA RN Lic, Valid Driver's license, clean driving record Common Equipment & Charting System: Clinic charting. no OASIS.
