Scope : This is an opportunity for a mission-driven leader who is
passionate about supporting our PACE participants with their social
work needs, and advocating for what matters most to each
individual. The Social Work and Advocacy Supervisor at the PACE
Center oversees the daily operations of our Social Work team,
Participant Advocates, and any behavioral health therapists. They
are relentless in understanding the needs of our participants,
advocating for our IDT to keep those needs at the forefront of each
interaction, and seeking effective ways to meet those needs. The
Social Work and Advocacy Supervisor provides leadership to their
direct reports, while also collaborating with fellow leaders to
ensure participants' physical, social, emotional, and recreational
needs are met. This position ensures high-quality, person-centered
care in alignment with PACE standards and regulatory guidelines.
Core Responsibilities & Expectations for the Role Contribute to a
center experience that Participants want to spend time in, a team
culture that cares and creates joy, and an environment where all
participants and team members belong. Continue to raise the bar.
Constructively seek and share feedback and help us implement
changes in order to improve clinical outcomes and experience for
participants. Exhibit and honor Habitat's values. Supervise one or
more Social Workers, ensuring high quality social services
delivered to our participants. Oversee our New Participant
Advocate(s), ensuring a strong welcome into our PACE program and a
focus on building love and trust during participants' transition
into the program. Serve as a primary liaison with community and
government resources on behalf of the PACE program. Serve as
escalation resource for participant issues (grievances, behavioral,
social work, engagement). Support team in care planning and driving
resolution of complex family dynamics / social work needs. Develop
and maintain staffing schedules and assignment of duties.
Collaborate with the interdisciplinary team (IDT) to assess
participant needs and update care plans. Support a culture of
safety, dignity and respect for all PACE participants and staff.
Performs related duties as assigned. Required Qualifications:
Masters in Social Work. Knowledge of PACE program or similar
healthcare operations. Knowledge of social work-related needs of
frail older adults, as well as knowledge of community and
government resources to meet those needs. Ability to be
self-directed and able to work independently. A minimum of two
years in a supervisory position. Proof of medical clearance for
communicable diseases, including a TB test. Proof of all
immunizations are up to date. Proof of current CPR/BLS
certification required or requirement to obtain within 30 days of
employment. Proof of valid CA driver's license, personal
transportation, good driving record and auto insurance as required
by State law. Preferred Qualifications: Licensed Clinical Social
Worker (LCSW). Bilingual: Spanish preferred. A state issued
driver's license, personal transportation, and auto insurance as
required by law. Location: Sacramento, CA (Onsite) Compensation: We
take into account an individual's qualifications, skillset, and
experience in determining final salary. This role is eligible for
medical/dental/vision insurance, short and long-term disability,
life insurance, flexible spending accounts, 401(k) savings, paid
time off, and company-paid holidays. The expected salary range for
this position is $90,000 - $116,000 and is bonus eligible. The
actual offer will be at the company's sole discretion and
determined by relevant business considerations, including the final
candidate's qualifications, years of experience, skillset, and
