Center work experience will start at the very beginning of the
salary scale. The Regional Center of the East Bay (RCEB) is one of
21 Regional Centers that are community-based, private non-profit
corporations under contract with the California Department of
Developmental Services. RCEB provides intake, assessment,
diagnosis, and coordinates community-based services for over 25,000
children and adults with developmental disabilities in Alameda and
Contra Costa counties. RCEB envisions a future where persons with
developmental disabilities are truly a part of the community.
Persons with developmental disabilities receive supports and
services they need to succeed. The supports and services change as
the need changes. In our dream, children with developmental
disabilities live with families in typical communities. Many choose
to attend their neighborhood schools and play with neighborhood
friends. Adults with developmental disabilities live in the
residence of their choice – with family members, with friends, or
with their spouse / significant other. They engage in activities of
their choice – work, volunteering, education, training, or leisure.
They have meaningful relationships with friends and coworkers and
are seen as valuable, contributing members of their communities.
Key Responsibilities : Case Managers are an integral part of
realizing our dream. Under general direction, Case Managers are
responsible for the entire case management process, including:
assessment and evaluation, planning, authorizing, advocacy,
monitoring, reviewing documentation for completion. Facilitate the
planning process and develop the Individual Program Plan (IPP) or
Individual Family Service Plan (IFSP) for assigned Consumers and
oversee the implementation of the IPP/IFSP. Monitor and evaluate
the services provided through the plan. Coordinate and support the
implementation of the Consumers plan which may include identifying
natural supports and generic services and evaluating when a need
exists that can only be met through RCEB authorized funding.
Advocate on behalf of consumers to protect their rights under the
law and ensure that they receive those services and benefits to
which they are entitled. Assist the consumer and family in
obtaining essential diagnosis and evaluation from appropriate
practitioners and secure written reports and other information
which assesses and interprets the consumer's situation. Monitor the
consumer's health and safety. Maintain all documentation and
contacts as required by law, regulation and policy. Implement RCEB
policies and procedures. Complete, in a timely manner, functional
and/or developmental reviews. Keep supervisor informed of the
status of the case load and unmet needs in the community. Perform
other duties as assigned. Minimum Qualification: Case Manager I BA
Degree in Social Work, Psychology or a related human services field
and 1 year similar experience. OR B.A. in an unrelated human
services field plus 4 years of experience working directly with the
developmentally disabled. Ability to communicate clearly and
effectively, verbally and in writing. Automobile and valid driver's
license required. Demonstrate cultural awareness and sensitivity
and an ability to work effectively with culturally diverse
populations. Ability to speak Cantonese and/or Mandarin fluently.
Case Manager II Master's Degree in Social Work, Psychology or
related human services field. Ability to communicate clearly and
effectively, verbally and in writing. Automobile and valid driver's
license required. Demonstrate cultural awareness and sensitivity
and an ability to work effectively with culturally diverse
populations. Ability to speak Cantonese and/or Mandarin fluently.
